It takes less than 2 minutes. Submit our online Transfer Rx form or call us at support@crmplatpharm.com — we'll contact your current pharmacy and move everything for you.
Do you offer free home delivery?+
Yes. We provide complimentary local delivery on maintenance medications throughout Miramar, Pembroke Pines, Hollywood, and surrounding Broward County areas.
Which insurance plans do you accept?+
We accept Medicare, Medicaid, and most major commercial insurance plans. We also accept Nation's Benefits and S3 Benefits OTC cards from plans including Aetna, Humana, Wellcare, and UnitedHealthcare.
What vaccines can I get at CRMPlatpharm?+
